Less than the framing suggests, and more in logistics than in medicine. A legitimate online TRT provider still runs blood work (you get it drawn at a local lab), still has a licensed clinician review and prescribe, and still monitors you over time - the consults just happen by video or messaging and the medication ships to you.
An in-person clinic does the same clinical work under one roof, with face-to-face visits. The core medicine - diagnosis, prescription, monitoring - is the same job either way; what changes is convenience, cost and how the care is delivered.
