Foundayo vs the Wegovy Pill (2026): Results, Price and How to Choose

The two FDA-approved GLP-1 pills compared: trial weight loss (OASIS 4 vs ATTAIN-1), the empty-stomach rule that only one of them has, published prices ($149-$349 at LillyDirect; $149 then $299 plus membership at ro), the $25 insurance offers and Medicare's $50 copay.

The quick answerBoth are FDA-approved daily GLP-1 pills. The Wegovy pill (oral semaglutide, approved December 2025) produced 13.6% mean weight loss at 64 weeks in OASIS 4; Foundayo (orforglipron, approved April 1, 2026) produced 11.2% at the top dose over 72 weeks in ATTAIN-1 - separate trials, not head to head. The Wegovy pill must be taken on an empty stomach with a 30-minute wait; Foundayo has no food or water rules. Self-pay, Foundayo lists $149-$349 a month by dose and the Wegovy pill $149 then $299 plus membership at Ro; both are $25 with a commercial savings card and $50 on Medicare's pilot.Ro's prices are its own published figures, verified August 2026. Manufacturer prices are as reported by the dated sources linked in the article and are not verified by us.

Key takeaways

  • Both are FDA-approved daily pills for chronic weight management: the Wegovy pill (oral semaglutide 25 mg, Novo Nordisk) since December 2025, and Foundayo (orforglipron, Eli Lilly) since April 1, 2026.
  • In their own trials - there is no head-to-head - the Wegovy pill produced 13.6% mean weight loss at 64 weeks (OASIS 4) and Foundayo 11.2% at the top dose over 72 weeks (ATTAIN-1), each against about 2% on placebo. Both companies also publish higher on-treatment figures (16.6% and 12.4%).
  • The practical difference is the routine: the Wegovy pill must be taken on an empty stomach with a sip of water and a 30-minute wait before eating; Foundayo can be taken any time of day with no food or water restrictions.
  • Price: Foundayo lists $149 to $349 a month self-pay by dose through LillyDirect and $25 with commercial coverage. The Wegovy pill is $149 the first month then $299 plus membership at Ro, with Novo Nordisk's reported direct tiers in the same range and a $25 commercial savings offer. Both are in Medicare's $50 GLP-1 Bridge.

What are Foundayo and the Wegovy pill?

They are the first two GLP-1 pills approved for weight management, and they are different kinds of molecule. The Wegovy pill is semaglutide - the same peptide as the Wegovy and Ozempic injections - in a 25 mg once-daily tablet, approved by the FDA in December 2025 for reducing excess body weight and maintaining the loss in adults with obesity, or overweight with a weight-related condition, and for lowering cardiovascular risk in that group. Because a peptide is poorly absorbed from the gut, the tablet carries a much higher dose than the 2.4 mg weekly injection and has to be taken in a specific way. Foundayo is orforglipron, a small-molecule (non-peptide) GLP-1 receptor agonist from Eli Lilly, approved on April 1, 2026 under the FDA's national priority voucher programme, 50 days after filing.

Being a small molecule is what lets it be taken without food or water rules. Both are prescription-only; neither is a compounded product.

embody offers doctor-prescribed GLP-1 weight loss treatment - compounded semaglutide and tirzepatide injections - through a 100% online process. Getting started takes a roughly 5-minute health qualifier, after which a medical practitioner reviews your chart (embody says usually within 24 hours, often under 5). Pricing is flat and simple: $69/month for semaglutide and $119/month for tirzepatide, with no insurance required, no hidden fees, and no clinic visits. Medication ships in 1-2 days in temperature-controlled, tracked, insured packaging, and embody is LegitScript-certified and works with US-based 503A compounding pharmacies. On Trustpilot, embody averages 3.8 across 8,398 reviews - an honest, mixed-to-positive record whose recent reviews cluster around two themes: responsive, proactive customer service ('from my very first inquiry I've had excellent communication... customer service updated me every step,' writes one tirzepatide patient) and the no-surprises pricing ('Love the fact that there are no surprises as in costly up front cost'). A 3.7 also means a minority of customers had negative experiences - worth reading both ends of the reviews before deciding, as with any provider.

altRx bills itself as "the #1 most affordable GLP-1 program," and its published prices largely back that up: compounded GLP-1 (semaglutide) from $89/month and GLP-1 + GIP (tirzepatide) from $149/month - flat at every dose, so the price doesn't climb as you titrate up - alongside one of the few genuine brand-name shelves in this space (Ozempic $1,149, Zepbound $1,249, Wegovy $1,579 per month). It's a self-pay telehealth service: you complete a short assessment, a licensed provider (physician, PA, or nurse practitioner) reviews it, and if appropriate a prescription goes to a licensed US pharmacy, with free shipping in about 5-7 days. No insurance is required, Buy Now, Pay Later is available, and you can pause or cancel anytime. Two things to know before the details: shipping is slower than the 1-2 day providers (embody, DirectMeds), and Trustpilot does not currently display an aggregate score on altRx's profile - the individual reviews we quote on this page are real, but there's no independent average to lean on the way there is for rivals like Medvi (4.3 across 14,836) or embody (3.8 across 8,398). One of the real 4-star reviews we surface notes a refill 'yet to be fulfilled with zero reason as to why' - worth factoring in alongside the price advantage.

Which pill produces more weight loss?

On the published trial figures, the Wegovy pill - but read the caveat before acting on it. The two drugs were tested in separate trials with different lengths and populations, and nobody has run them head to head.
TrialDrug and doseMean weight changeLost 20% or more
OASIS 4 (64 weeks, 307 adults)Wegovy pill, 25 mg daily-13.6% vs -2.2% placebo30% vs 3%
ATTAIN-1 (72 weeks, 3,127 adults)Foundayo, 36 mg daily (top dose)-11.2% vs -2.1% placebo19% vs 3%
Those are the figures from the New England Journal of Medicine papers, counting everyone who was randomised whether or not they stayed on the drug. Both manufacturers also publish an on-treatment figure - 16.6% for the Wegovy pill and 12.4% for Foundayo - which describes people who took the pill as directed to the end; you will see both sets of numbers quoted, and they are not contradictory, just different questions. Two further context points: the OASIS 4 authors describe the pill's effect as similar to the weekly semaglutide injection in its own trial, and injectable tirzepatide (Zepbound) still produced the largest average loss of any GLP-1, up to 22.5% in SURMOUNT-1. Pills vs injections covers that comparison.

How do you take Foundayo and the Wegovy pill?

This is the difference most people will feel every day. The Wegovy pill is taken once daily on an empty stomach, with a small sip of water, and you then wait about 30 minutes before eating, drinking or taking other medicines; the dose is stepped up over the first months from a 1.5 mg starting tablet to the 25 mg maintenance dose. Miss the timing and absorption drops, which is why the rule exists. Foundayo is taken once daily at any time, with or without food or water, and is stepped up across 6 mg, 12 mg and 36 mg tablets.

For someone who takes morning medication, drinks coffee on waking or simply does not want a timed routine, that is Foundayo's main argument. Both are started low and increased under a clinician's supervision, and both are long-term treatments: in the GLP-1 trials, weight returns when the drug stops.

What do Foundayo and the Wegovy pill cost?

Close to each other at the entry price, with different structures behind it. ro's figures below are its own published prices, verified by us in August 2026. The manufacturer prices are as reported by the dated sources linked, and the NovoCare tiers in particular come from third-party trackers that do not fully agree with each other - confirm them with NovoCare before budgeting.
RouteFoundayoWegovy pill
Manufacturer direct, self-pay$149-$349 a month by dose through LillyDirect, as reported by Scientific American from Lilly material$149-$299 a month by dose through NovoCare Pharmacy, as reported by third-party trackers; not verified by us
Telehealth (verified)Lilly says Foundayo is available through LillyDirect and telehealth providers; none of the providers we track has published a price we could verifyro: $149 first month, then $299, plus membership ($39 first month, then $74-$149)
Commercial insurance with coverage$25 a month with Lilly's savings card, per Lilly's April 9, 2026 releaseas low as $25 a month with Novo Nordisk's savings offer, capped at $100 of savings per 30-day supply (reported)
Medicare GLP-1 Bridge$50 flat copay, July 2026 to December 2027$50 flat copay, July 2026 to December 2027
The gap that matters is at the top dose: Foundayo's reported ceiling is $349, the Wegovy pill's $299. The gap that matters more is whether your insurance covers either one, because $25 beats both. Wegovy pill price online walks the semaglutide side in full.

What To Know Before Starting GLP-1 Treatment

Before starting GLP-1 treatment, it's important to understand a few key things. These medications require a prescription from a licensed clinician and aren't suitable for everyone. Some people may experience side effects, especially during the first few weeks as the body adjusts.

GLP-1s also tend to work best when combined with basic lifestyle habits like balanced nutrition and regular physical activity.

That's why choosing a provider that offers proper medical screening and ongoing follow-up support is essential for both safety and long-term success.

Do insurance and Medicare cover Foundayo and the Wegovy pill?

Both drugs sit inside the same two programmes. Commercial insurance. Each manufacturer offers a savings card for commercially insured patients whose plan covers the drug - Lilly's bringing Foundayo to $25 a month, Novo Nordisk's reported to bring the Wegovy pill to as low as $25 - and neither card is available on Medicare, Medicaid or other government plans. Coverage itself is the hurdle: plans still vary widely on weight-management GLP-1s, and our insurance guide explains how to check before you choose. Medicare. The Medicare GLP-1 Bridge, a CMS pilot running July 1, 2026 through December 31, 2027, covers the Wegovy pill and injection, the Zepbound KwikPen and Foundayo at a flat $50 copay that does not rise with the dose. If you are on Medicare, that single question - am I eligible for the Bridge - matters more than anything else on this page; GLP-1s after 65 covers what to ask your Part D plan.

What are the side effects of Foundayo and the Wegovy pill?

The same family as every GLP-1: mostly gastrointestinal, mostly early, mostly during dose increases. In OASIS 4, gastrointestinal adverse events occurred in 74% of people on the Wegovy pill against 42% on placebo, led by nausea and diarrhoea.

In ATTAIN-1, gastrointestinal events were also the most frequent with Foundayo and were typically mild to moderate. Neither trial reported a safety signal outside the known class profile, which carries the standard GLP-1 warnings around thyroid C-cell tumours in rodents, pancreatitis and gallbladder disease.

Dose and how gradually you step up matter more than which pill you take, and a clinician who manages the titration is the single biggest factor in tolerating either - our semaglutide side-effects guide covers what helps.

Which should you choose, Foundayo or the Wegovy pill?

Work down the list. Your insurance covers one and not the other: take the covered one; at $25 a month the difference between them is a rounding error. You are on Medicare: both are in the Bridge at $50, so choose on routine and results, not price. You want the larger trial result: the Wegovy pill, on the published figures, with the caveat that the trials are not directly comparable. You cannot or will not keep an empty-stomach, 30-minute-wait routine every morning: Foundayo, which has no timing rules at all. You are paying cash and want a verified telehealth route today: the Wegovy pill through ro is the route we have checked; for Foundayo, LillyDirect's published self-pay price is the reference. You are needle-averse but price-led: read what $100-$200 a month gets you first, because the compounded injections at $49-$199 sit well below either pill at cash prices. Whatever you pick, the prescriber decides on your history, not on a table.

The bottom line

Foundayo and the Wegovy pill are the two FDA-approved GLP-1 pills for weight management, and they split cleanly: the Wegovy pill has the larger published trial result (13.6% vs 11.2%, different trials) and Foundayo has the simpler routine (no food or water timing). On price they overlap - $149 at the starting dose from either manufacturer as reported, $25 with commercial coverage, $50 on Medicare's pilot - with Foundayo's reported top-dose price $50 higher. ro's $149-then-$299-plus-membership is the one telehealth price here we have verified.

Both are prescription medicines; a licensed clinician decides whether either is appropriate for you. This article is general information, not medical advice.
